If you need to cancel an appointment, please either:
- fill out the form
- use the app
- call the surgery
Should you need to re-book your appointment, please call the surgery.
Submitting a Patient Triage Form
If you would like an appointment with the GP, you must use the link below to submit a patient triage form. It will ask you some questions about the issue you wish to discuss. This will then be triaged by a GP, and you will be contacted with the outcome. Please make sure to your phone available. We aim to contact you within 24 hours of receiving your form, often it is much quicker. We do try book the majority of patients on the day, so please make sure you are available on the day you submit an econsult.
Triage form opening hours are 8am to 4:30pm, Monday to Friday. Please only call the surgery after 4:30pm if your matter is urgent for that day, otherwise please complete a triage form the next working day. If you have additional needs, which means you are unable to complete the online form, please speak to reception, who can complete a version of the online form on your behalf.
Please remember that we are not an emergency service. If you experience any symptoms such as: severe chest pain, breathlessness, stroke symptoms or are in a life-threatening emergency, please call 999.
If you need help when we are closed and its not as urgent as 999, please call 111 or visit their website.
Home Visits
If you require a home visit, please complete a Patient Triage Form via econsult or please call the surgery as soon as possible.
