Online 'Pre-Registration' With The Practice

PreRegistrationIf you wish to register with the Practice, please download the relevant questionnaire below, and bring it together with the GMS 1 registration form to the surgery - both must be completed.  Separate forms are required for each registration. We ask you to complete a medical questionnaire because it can take a considerable time for us to receive your medical records.  We also ask all patients over 16years to use our blood pressure machine for an up to date recording on registration.


Please remember that to register with the surgery we must have a completed new patient questionnaire AND a GMS1 form for each person.

ONLINE PATIENT SERVICES (EMIS online patient access)  

This facility offers registered patients over the age of 16 years access to repeat prescription ordering, GP appointment booking/viewing/cancelling, and a partial view of your clinical record such as allergies and immunisations. 

Once registered as a patient, you can then register to use this online access service.  Please ask for details at reception when collecting or dropping in your new patient registration form(s), or complete the slip attached to the questionnaire.  We will need to see a form of ID such as a driving licence or passport to verify your identity.

Once you are registered as a patient, you will then be sent online services registration information by email which tells you how to complete the online registration process.
